Excessive alcohol use (greater than 40 grams per day in women and greater than 60 grams per day in men) in patients with chronic HCV infection has been associated with accelerated fibrosis progression. A clinician’s Pocket Guide for Alcohol Screening and Brief Intervention is available from the National Institute on Alcohol Abuse and Alcoholism (NIAAA) to inform clinicians how to take an appropriate alcohol history. Alcohol Use: Detailed quantitative information should be obtained from the patient regarding current and past alcohol use.Once an individual is diagnosed with cirrhosis, there are key elements of the history, physical examination, and laboratory studies that need to be addressed and monitored. Some experts have proposed a 4-stage cirrhosis classification system that risk stratifies individuals according to the presence of ascites, esophageal varices, and variceal bleeding to differentiate and stage compensated and decompensated disease ( Figure 3). In general, any person with decompensated cirrhosis should receive evaluation and medical care by a hepatologist or liver diseases specialist. In addition, the presence of decompensated cirrhosis can have major implications regarding management and prevention of cirrhosis-related complications, as well as the potential need for a referral for liver transplantation evaluation. Prognosis and survival are markedly better in persons with compensated cirrhosis than in those with decompensated cirrhosis ( Figure 1) and ( Figure 2). Distinguishing Compensated versus Decompensated Cirrhosis In contrast, persons with decompensated cirrhosis often have symptomatic complications related to cirrhosis, including those related to hepatic insufficiency (jaundice or hepatic encephalopathy), and those related to portal hypertension (ascites or variceal hemorrhage). Persons with compensated cirrhosis often do not have signs or symptoms related to their cirrhosis, although they may have evidence of portal hypertension, such as esophageal or gastric varices. Once it has been established that an individual has cirrhosis, it becomes very important to determine whether they have compensated or decompensated cirrhosis. Defining Compensated and Decompensated Cirrhosis As individuals develop more extensive hepatic fibrosis, physiologic complications can develop, such as increased pressure within the portal system, disruption in bilirubin metabolism, and reduced production of coagulation factor proteins. Advanced fibrosis and cirrhosis-at their early stages-are not usually clinically detectable or symptomatic. In an estimated 20 to 30% of persons with hepatitis C virus (HCV) infection, chronic viremia results in inflammation, followed by fibrosis and cirrhosis.

MDMA was found to induce significant and robust attenuation in CAPS-5 score compared with placebo (P < 0.0001, d = 0.91) and to significantly decrease the SDS total score (P = 0.0116, d = 0.43). Adverse events and suicidality were tracked throughout the study. PTSD symptoms, measured with the Clinician-Administered PTSD Scale for DSM-5 (CAPS-5, the primary endpoint), and functional impairment, measured with the Sheehan Disability Scale (SDS, the secondary endpoint) were assessed at baseline and at 2 months after the last experimental session. After psychiatric medication washout, participants (n = 90) were randomized 1:1 to receive manualized therapy with MDMA or with placebo, combined with three preparatory and nine integrative therapy sessions. We report the findings of a randomized, double-blind, placebo-controlled, multi-site phase 3 clinical trial ( NCT03537014) to test the efficacy and safety of 3,4-methylenedioxymethamphetamine (MDMA)-assisted therapy for the treatment of patients with severe PTSD, including those with common comorbidities such as dissociation, depression, a history of alcohol and substance use disorders, and childhood trauma. Boondocking is More About Self-Sufficiency So, the term “boondocking” came up as a way to differentiate between these two perspectives. They prefer to stay in civilization, but want the mobility and freedom of RVing. People who never enjoyed tent camping, but later went on to purchase an RV, usually don’t drive their rigs deep into the woods or desert. Their interest is more focused on seeing new sights, trying new restaurants. Hence, RV boondocking often stems from this standpoint.īy contrast, RVers who primarily stay in RV Parks and developed campgrounds are travelers who prefer to take their “hotel room” with them from place to place. From that perspective, they still wanted to enjoy to sights and sounds of nature, but wanted something more comfortable than a tent. Eventually, they became drawn to small trailers that they could pull with a car. Over the years, as hikers got older and slower, they found their bodies aching from the cold and cramped quarters of a two-person tent. The female XLR connectors are designed with a longer metal sleeve to first connect pin 1 (the earth pin), before the other pins make contact, when a male XLR connector is inserted. This is slightly unusual as many other connector designs omit one of the styles (typically a chassis mounting male connector). XLR connectors are available in male and female versions in both cable and chassis mounting designs, a total of four styles. Later, the Switchcraft corporation manufactured compatible connectors, followed by the Neutrik company, which made improvements to the connector, and produced a second-generation design (the X-series) that had only four parts for the cable connector, and eliminated the small screws used in the models of XLR connectors made by Cannon and Switchcraft. The Australian factory was sold to Alcatel Components in 1992 and then acquired by Amphenol in 1998. Originally, the ITT Cannon company manufactured XLR connectors in two locations: Kanagawa, Japan, and Melbourne, Australia. There was also the XLP series of connectors with hard plastic insulation, but the XLR model name is commonly used for all of the variants. The XLR connector originated from the Cannon X series of connectors by 1950, a latching mechanism was added to the connector, which produced the Cannon XL model of connector, and by 1955, the female connector featured synthetic-rubber insulation polychloroprene (neoprene), identified with the part-number prefix XLR. Cannon, founder of the Cannon Electric company, Los Angeles, California. The XLR connector (also Cannon plug and Cannon connector) was invented by James H. History and manufacturers Male and female XLR connectors with different numbers of pins The XLR connector is superficially similar to the smaller DIN connector, with which it is physically incompatible. XLR connectors are included to the international standard for dimensions, IEC 63. XLR connectors are cylindrical in design, with three to seven connector pins, and are often employed for analog balanced audio interconnections, AES3 digital audio, portable intercom, DMX512 lighting control, and for low-voltage power supply. The XLR connector is a type of electrical connector primarily used in professional audio, video, and stage lighting equipment. XLR3 cable connectors female (left) and male Style of electrical connector found primarily in professional audio and lighting HDMI audio extractors can be divided into two large groups – those with audio outputs only (optical, coaxial, AUX, RCA) and those with audio and video (HDMI) outputs. In this case, the extractor serves as a switch between different devices. Since most projectors don’t have audio extractors or, in some cases, they have only AUX or RCA audio outputs, you need an audio extractor to separate the signal coming from the Blu-ray player, and then send video to the projector and send audio to your AV receiver (or to your powered stereo speakers).Īlso, if you want to connect all kinds of equipment (gaming consoles, TV sticks, cable boxes, etc.) to your TV but don’t have enough HDMI inputs, using an extractor with multiple HDMI inputs and one HDMI output, is an affordable and efficient solution. HDMI audio extractor can come in handy when building a large home theater system with a Blu-ray player, projector, and a 5.1 surround sound speaker system (or a powered stereo speaker system). The characteristics and capabilities of the extractors mostly depend on the price. Also, some extractors output only stereo sound while other extractors can output both stereo and surround sound. Some HDMI extractors can output both audio and video, while others output only audio. They can separate the audio from the video and send it to the audio output device (like a speaker system) or to some other device that has no HDMI inputs ( AV or stereo receiver, Bluetooth transmitter, etc.). These extractors are connected to the HDMI audio source. The purpose of HDMI Audio extractors is pretty simple. What Is The Purpose of The HDMI Audio Extractor? This is the parallax effect in action: Image Credit: Brit Cruise. You will find that your finger jumps back and forth (or splits into two). For example, pick up means to grab or lift, very different from the definitions of pick and up alone. While looking at the background alternate opening your left and right eye one at a time. Phrasal verbs are two or more words that together act as a completely new verb with a meaning separate from those of the original words. For stars more than about 100 light-years from Earth, we cannot measure any shift and the method fails. Hold up a finger and focus on something in the distant background. The ratio of the Moon’s mass to that of Earth is 1/81.30, and thus the ratio of Earth’s mass to that of the Sun is found. The farther the star is, the smaller the angles. The ratio of the combined mass of Earth and the Moon to that of the Sun may be determined from the disturbing action of Earth and Moon on the elliptic motion of the planets. This is shown in the image below.įrom the image above, you can see that by knowing the size of Earth's orbit and measuring the angles of the light from the star at two points in the orbit, the distance to the star can be derived. Now consider that the Earth moves in its orbit around the Sun, allowing us to look at nearby stars from slightly different locations - just like your two eyes are at slightly different locations. What was different this time? This is a demonstration of the parallax effect: the apparent shift in position of a relatively nearby object against more distant ones when viewed from different vantage points. What do you notice? Move your thumb closer to your face and repeat the experiment. Now look at your thumb with your other eye. In astronomy the term is used for several techniques for determining distance. Hold out your thumb at arm's length, close one of your eyes, and examine the relative position of your thumb against other distant (background) objects, such as a window, wall, or tree. parallax (prlks), any alteration in the relative apparent positions of objects produced by a shift in the position of the observer. You are probably familiar with the phenomenon known as parallax. This method that relies on no assumptions other than the geometry of the Earth's orbit around the Sun. Astronomers derive distances to the nearest stars (closer than about 100 light-years) by a method called stellar parallax. |
